    Dr. Bao Nguyen Dang

Locally born, raised and schooled in Perth, I graduated from the University of Western Australia in 2005. After several years rotating through the public hospitals including Sir Charles Gairdner Hospital, Princess Margaret Hospital, Joondalup and Armadale Health Campuses as well as working with Silver Chain Palliative Care service, I gained very valuable experiences across many specialties including
emergency medicine, paediatrics, antenatal care, oncology and palliative care and general medicine. This set me up with much more confidence and a wondering foundation entering general practice. I am very privileged to have been a part of the Third Avenue Surgery family since 2011.

I love the challenges of being a GP, with all its diversity as well as the holistic nature of care that we can provide. My particular interests lie in paediatrics, women’s health and pregnancy care, preventative health and chronic illness. My overall aim is to be your partner in helping you and your family to achieve improved mental and physical well being.

Outside of work, my three greatest loves include my beautiful fluffy Maltese Shih Tzu Wasabi, my lil miss Lily and my baby Oliver. I also delight in delectable food, shoes (the more the merrier) and travel.

    Dr. Ben Walawski

Ben is currently on extended leave.

Growing up in Perth and completing my degree at the University of Western Australia, I went on to work for a number of years at Royal Perth Hospital and overseas at Galway University Hospital. Upon returning to Perth, I spent time at King Edward Memorial Hospital working in obstetrics before
completing my Diploma of Child Health at Princess Margaret Hospital. Over the years, I have developed a keen interest in paediatrics and women’s health.

Following completion of my specialist training in General Practice, I have been fortunate enough to split my time between work and raising my two young sons. We enjoy camping, cycling and getting busy in the shed working with wood.

I have worked with the wonderful team of doctors and staff of Third Avenue Surgery since 2011 and I thoroughly enjoy the opportunity to care for my patients. I am humbled by the trust parents put in me to care for their children and delight in fostering long term relationships as much as providing quality medical care.

  • Profile image of Dr. Christopher Bleakley

    Dr. Christopher Bleakley

I grew up in Belfast and attended Queen’s University, graduating in 2001. I completed most of my hospital training around Northern Ireland, with a year in Brisbane for good measure, before moving to Chester, England to complete training as a GP in 2007. I became a partner in a surgery in Belfast in 2009, and started working as one of the local hospice doctors in the same year.

After 34 years of rain (minus one in Brisbane), I decided that I’d had enough and moved to Perth with wife and (then one) child. My wife is also a GP, and we worked together in a bulk-billing surgery in Midland for 2 years. I made the decision to move to private billing surgeries at that time, because it allows me to spend more time with each patient and to provide higher quality health care – our main goal on leaving the NHS. I have now been working at Third Avenue Surgery for a few years, and also work for the GP After Hours folks when I can.

I am interested in all aspects of General Practice and am passionate about the role of the GP in preventing illness before it causes harm. I don’t find it helpful to drag well people in to see the doctor, but am very keen to provide support and health education so that, should things start to go wrong, we can jump on any problems quickly and effectively.

I am a fair weather cyclist (in recognition of the fact that we should at least try to lead by example). Beyond this, I am something of a nerd (and proud of it), spend most of my time trying to entertain/distract two children, and give thanks for the wealth of family-friendly activities that are available in and around Perth.

    Dr. Christopher Cook

I live in the local area with my wonderful wife and two amazing children. I am interested in music, travel, martial arts and have been known to occasionally spoil a nice walk on the golf course.

In my life before medicine, I completed a degree in Sports Science and worked at Concord Hospital, Sydney, in the Respiratory Function Unit. I soon realised the doctors were having all the fun so I applied to study medicine at UWA.

Since graduating from UWA in 2007, I completed my internship at Royal Perth Hospital and then worked around Perth in a variety of specialties at St John of God Hospital, Hollywood Private Hospital and Joondalup Health Campus. Following GP training and Fellowship, I have completed a Diploma in Dermatology and Certificate in Skin Cancer Medicine. I am particularly interested in skin disease and perform comprehensive skin checks and minor skin procedures daily.

At Third Avenue Surgery, we provide travel consultations and vaccinations. I would like to hear all about your future travel plans and help minimise your risk of health problems whilst abroad.

I enjoy all aspects of General Practice including preventative health care, sexual health and contraception – including Implanon insertion and removal, and chronic disease management. I provide a relaxed, collegial approach to health care where we work together to optimise your health.

I look forward to seeing you at Third Avenue Surgery.

    Dr. Penny Roe

I grew up in Sydney but moved to Newcastle to study medicine where I graduated with Honours and the AMA prize. I then returned to Sydney and completed my hospital training at Prince of Wales Hospital and Sydney Children’s Hospital doing paediatrics.

I completed a Diploma of Child Health followed by the Family Medicine Program and became a Fellow of College of GP after passing the exams in 1996. I had my son Tom in 1997 but continued to work in General Practice in the inner west of Sydney before moving to Perth in 2003 for family reasons. I have worked at Third Avenue Surgery since 2006 and for GP After Hours Mount Lawley for over 10 years.

I love all aspects of General Practice and Family Medicine but I am especially interested in Paediatrics,
Women’s Health and younger adults. I am passionate about the role of the good GP in a person/family’s life and the importance of continuity of care for patients. That is why I am so fortunate to work with so many great GP’s here at Third Avenue Surgery and GP After Hours Mount Lawley.

My grown son Tom has almost finished university and we have a boxer dog named Tessa and two cats. I love walking Tessa, waging a losing battle on the weeds in my garden, book club, movies and enjoying lots of social events with friends, family and colleagues.

I grew up in Mount Lawley where I attended Coolbinia Primary School and Mt Lawley High School so being able to also work in Mount Lawley has been very satisfying for me. I graduated from UWA medical school in 1990 and have worked as a General Practitioner since 1995.

My areas of special interest are paediatrics and acupuncture for weight loss and pain control. I am a
member of the Australian Medical Acupuncture College and therefore Medicare rebates are available on this treatment.

I have been a part of the team at Third Avenue Surgery since 1999. I am very passionate about my work and am available Monday through to Friday. As I offer a wide range of appointments, including same day if required, my patients are able to book ahead for a day that suits them.

    Dr. Louis Papaelias

I was born in Perth and grew up locally. I studied medicine at the University of WA and graduated in 1975. I went on to complete additional studies where I became a Fellow of the Royal Australian College of GP’s and have practised as a General Practitioner since 1979.

I have been with Third Avenue Surgery since 1983, where I began as one of the original founding doctors alongside Alla Sakalo. I have completed additional studies in Obstetrics and delivered babies as a GP Obstetrician for many years. At present, my main area of interest is in all aspects of general practice. I visit aged care facilities to look after my patients twice a week and I am also fluent in Greek and basic Italian. I also enjoy working for GP After Hours Mount Lawley, our division of after-hours emergency care.

I am a member of the AMA and RACGP and my deep love of wine has led me to write the occasional wine article for the Medical Forum Magazine. I am happily married with two grown children and one grandson and in my spare time I enjoy reading, listening to music, occasional cooking and roasting coffee and weekend solitude time in our holiday home in Denmark where I can ‘clear my head’.

  • Profile image of Dr. Michelle Cotellessa

    Dr. Michelle Cotellessa

I completed my degree in Bachelor of Medicine and Surgery in 1994 at UWA. Since graduating, I have worked at Royal Perth Hospital in many different areas of specialty, but my love of women’s health led me to King Edward Memorial Hospital where I completed my Diploma in Obstetrics and Gynaecology. I have also completed 4 years of training in General Practice and gained my fellowship in 1999.

Over the years, I have developed special interests in women’s health including antenatal, post natal,
contraception and menopause, mental health, preventive and general medicine, but I thoroughly enjoy the wide spectrum of general practice. A very important part of general practice is ongoing education which is something I enjoy and continue at every opportunity.

I have worked in some wonderful general practice settings and since 2010, have had the privilege of working with the amazing team at Third Avenue Surgery. It is an honour and an absolute pleasure to work in General Practice and to develop long term relationships with patients. I am a wife and a mother to two children, Emily and Joshua, who keep me very busy with their different activities. Our family wouldn’t be complete without our German Shepherd Lola, who enjoys taking me to the park. In my spare time I enjoy time with family and friends, reading and keeping active.

  • Profile image of Dr. Simon Torvaldsen

    Dr. Simon Torvaldsen

I was born in England and migrated to Perth, aged 12 and studied Medicine at the University of WA. I spent three years at Fremantle Hospital, and was then appointed Medical Registrar to the Palliative Care Unit at Hollywood Hospital. I then worked as a GP locum in both metro and country areas and in 1991 I settled at Third Avenue Surgery and continued to work part-time with Silver Chain Hospice
    for 16 years.

I love all aspects of General Practice including procedural and family medicine and have extensive experience in Aged and Palliative Care. Currently, I am Chair of the AMA WA Council of General Practice, where I enjoy working to improve our health system and promote the value of General Practice.

I take great satisfaction in trying to make a difference both for my patients and for the future of medicine in Australia. Over the years, I have sat on various Boards of Divisions of General Practice and Medicare Locals.

I founded Australia’s first private Palliative Care Unit at Mount Lawley Private Hospital and was a founding member of the Australian Motor Sport Medicine Association.  In 2000, I helped create GP After Hours Mount Lawley at Mercy Hospital and am proudly responsible for its relocation to Third Avenue Surgery. I have published in the Medical Journal of Australia and have provided expert
advice to a number of Government committees, the Medical Board and the media.

I am happily married with two grown daughters and when I do have spare time, I am a keen offshore sailor.

  • Profile image of Dr. Tim Leahy

    Dr. Tim Leahy

I was raised here in Mount Lawley and attended UWA for my undergraduate medical degree. I completed my Masters in Family Medicine from Monash University and my Fellowship with the College of General Practitioners, and am currently doing further study in medical education at the University of Notre Dame in Fremantle, which helps me with my part-time work there as a medical educator.

I’ve worked at Royal Perth Hospital and in various agencies around Perth. I’ve previously worked in suburban general practices in Lockridge and Bayswater, 7 years at the DerbarlYerrigan Health Service as well as a stint at the Aboriginal health service in Roebourne. I’ve worked at Third Avenue Surgery since 2002 and count myself very lucky to work with great colleagues and lovely people who choose us to care for them.

My interests in General Practice are varied and cross the lifespan. I am interested in supporting individuals and families with the many common and not so common challenges that are part of life. I realise whether the problem is large or small, simple or complicated by other problems, the patient and their family have their own ideas, concerns and feelings and they will often want to choose from the best available evidence based options and access services that support them. I am keen to assist wherever I can.

Aside from work, we are blessed to have two boys who regularly remind me of the modern day challenges of parenting. They are more than happy to help me fill in any spare time I have and when not with them, I try to swim and get out and about for a coffee.

  • Profile image of Dr. Sunita Chelvanayagam

    Dr. Sunita Chelvanayagam

Raised and schooled in Perth, I attended medical school in Dublin, Ireland. I returned home to work for some time at Royal Perth Hospital pursuing a surgical career until I met my husband.  With the prospect of family ahead of me I decided I wanted to be a General Practitioner.

After completing my fellowship, I worked for many years in Kalamunda. After having my second daughter, I commenced working at Third Avenue Surgery which was closer to home and where I have been since 2013. I thoroughly enjoy working and living in the community and contributing to its wellbeing.

My special interests include Women’s Health and Sexual Health, Pregnancy care, Paediatrics and Mental Health. I enjoy doing minor procedures including Implanon and Mirena insertion
as well as removal of skin lesions. I also regularly work at GP After Hours Mount Lawley, our division of after-hours care.  My philosophy with patients is to have an open and comfortable relationship as much possible and to treat them as a whole person. Being a mother of two school aged daughters, I understand what is involved in looking after a family and value the input I can have in caring for many of our patient’s families. I am very honoured to be working for a practice that values high standards of medical care, ongoing medical education and peer support.

  • Profile image of Dr. Gwendolyn Wu

    Dr. Gwendolyn Wu

I grew up in Singapore and came to Australia to further my tertiary education following which I then completed my medical degree at the University of Queensland. I then decided to move to Perth to do my internship, have a change of scenery and be closer to family.

Working in several hospitals and areas including Royal Perth Hospital, King Edward Memorial Hospital and Princess Margaret Hospital helped me to make the decision that General Practice was the path for me. The ability to care for patients in a holistic manner, across generations from birth to end of life is what pulled me to General Practice and is what I enjoy doing to this day.

I enjoy all aspects of General Practice and am particularly drawn to Paediatrics, Travel Medicine, Women’s Health and Antenatal Care and Palliative Care. A good work life balance, I believe is important to being a good practitioner and I find travelling, photography and hiking are amongst the many things that I try to do that helps me keep this balance.

I have found it to be a privilege working with the group of dedicated and passionate Doctors at Third Avenue Surgery and I am glad to be able to share with them their vision to continue to provide the best care possible to our community.

  • Profile image of Dr. Jane Purdie

    Dr. Jane Purdie

After graduating from University of WA, I spent 7 years working in Royal Perth Hospital with the idea of specialising. I made the decision to switch to General Practice and have not regretted this career change.

I joined the team at Third Avenue Surgery in 1996, initially working part-time as well as working for Silver Chain Hospice. I currently work here full-time. I like all aspects of general practice. As a female doctor you have to have an interest in women’s health but I also like medicine as it applies to aged care and paediatrics. Mostly I like the ability to care for patients over an extended time which enables me to know and understand their problems. I feel privileged to have a job which enables me to actually help people.

I have previously spent all my spare time, since I was 16, playing water polo. I have officially hung up my bathers and exchanged them for a set of golf clubs. If I get the opportunity, I would add a camera to my list of interests as well.

  • Profile image of Dr. Tammie Wong

    Dr. Tammie Wong

I was born and raised in Perth, Australia and obtained my Bachelor of Medicine and Surgery from the University of Notre Dame Fremantle in 2010.

Prior to Medicine, I had completed a Bachelors of Biomedical Science and Molecular Biology with Honours. It was through my Honours year that I learned (rapidly) that I much prefer working with people than in a laboratory.

I trained at Sir Charles Gairdner Hospital, Joondalup Hospital and Hollywood Hospital. I discovered that I enjoy the ongoing therapeutic relationship with patients and General Practice was definitely for me. I consider myself very fortunate to have joined the 3rd Avenue Surgery team at the end of 2018.

My interests in General Practice include Women’s Health, Sexual Health, Chronic Disease Management and Mental Health. I have an interest in Dermatology and currently am undergoing more training in this area.

Outside of work, I enjoy the beach, the outdoors, hiking and jogging. I love food and travel. I am an animal lover and have a gorgeous rescue Greyhound and cat.

  • Profile image of Dr. Bhawani (Bu) O’Brien

    Dr. Bhawani (Bu) O’Brien

I was born in Malaysia and am of Sri Lankan descent. Having parents who were Doctors led to my early interest in a career in Medicine.

After primary school education in Malaysia, I spent my secondary school years as a boarder in Cheltenham, England and then went on to study Medicine in Dublin, Ireland.

I enjoyed my junior doctor years in Scotland working in various specialties and gaining my Diplomas in Child Health and Obstetrics & Gynaecology. Those years helped me decide on my chosen path of General Practice. I loved the relationships formed with the continuity of care and the privilege of helping people from birth to end of life.

I completed my training in General Practice and passed my Fellowship exams in Birmingham, England before moving with my young family in 2008 to sunny Bunbury. I have spent the last 12 years as a GP in Bunbury and also worked for 5 years as a Breast Physician in the Bunbury branch of Breastscreen WA. I held the positions of director and chairperson of the Koombana Health Network and also examined medical students for the rural clinical school. I have also been involved with Headspace Bunbury and the South West Cancer Strategic team.

My move to Perth was prompted by my children’s tertiary education, and I now find myself in a fortunate position of working with an inspiring and supportive team at Third Avenue Surgery. I hope to form some lasting bonds with my new patients and families as we work together to keep you in good health.

‘All that really matters is that we are healthy and happy – everything else is just sprinkles on the sundae.’
