Chronic Care

Chronic Conditions/Diseases were previously managed through GP Management Plans and Team Care Arrangements. As of 1st July 2025, the Australian Government has made changes in the structure and delivery of the management of chronic conditions. GP chronic condition management plans are intended for patients that would benefit from a structured approach to managing their chronic condition(s). These plans allow GPs to work with their patients to set the goals for treatment/management of their condition, roles and responsibilities, and a structured plan for their care. These plans may also make provision for support from Allied Health Practitioners if appropriate.

What you need to know

1. Your GP will aim for quarterly reviews of your management plan. These reviews are designed to be streamlined and more efficient and generally shorter than previous care plans. More regular reviews help monitor your progress towards your goals and identify any changes to health needs.
2. Our practice nurses will still be involved with your care with up to 5 services provided to support management of your chronic condition.
3. You are eligible for a GP chronic condition management plan if you have at least one medical condition that has been or is likely to be present for at least 6 months.
4. Our admin team will book you in for your next review with a 3 month interval.

Acute problems or any issues not related to your chronic condition will need to be addressed at a separate appointment.
